Your privacy matters. Before or during your treatment, Sunny Skies Healthcare’s TeamCare may need to review your past and current medical records to provide the safest, most effective care possible. To do this, we ask patients to complete a HIPAA Authorization Form. This form gives your permission for our providers to request and receive your health information from other doctors, clinics, hospitals, therapists, or healthcare facilities that have treated you.
Why the HIPAA Authorization Form Is Important
The HIPAA Authorization Form helps our TeamCare fully understand your medical history before starting or continuing treatment. Having accurate records allows the care team to review your previous diagnoses, medications, treatments, test results, provider notes, and other important health information that may affect your care plan. This also helps us create a more personalised treatment plan, coordinate with your other healthcare providers, avoid repeating unnecessary tests, and reduce the risk of missing important details that could impact your treatment.
What Information May Be Requested
With your permission, Sunny Skies Healthcare may request relevant health information from providers or facilities you choose. This may include medical records, mental health records, medication history, treatment notes, lab results, previous evaluations, hospital records, therapy records, or other information needed to support your care.
The goal is not to collect unnecessary information. The goal is to make sure our providers have the details needed to understand your health history and make thoughtful treatment decisions.
What You Control
You remain in control of your health information. You decide what information Sunny Skies Healthcare can request, which providers or facilities we can contact, and what records may be shared with our TeamCare. You may also cancel this authorization at any time by notifying us in writing. Cancelling the authorization does not affect information that was already shared before the cancellation, but it can stop future requests or disclosures based on that authorization.
